Online Application Forms

This site is a tool to speed up access for reservations, accessing forms and submit request in a better way. If you do not find what you are looking for or have questions, use our Ask Us form to send us your question.

• AIC Student Employment Application
  Library job request forms.
• EMail AIC
  For any information that is needed
• Portable Smart Classroom Unit Request Form
  A full Portable Smart Classroom Unit may be checked out by an instructor for the entire semester. Questionnaire is needed so that relevant datasets, lectures, etc., may be loaded.
• New Materials Request
  Request for new materials to be added to the AIC collection.
• TRAINING ROOM RESERVATION REQUEST
  To request the AIC training Room for Workshops and Training classes.
