Using the Standard In-Service Evaluation Form

The Committee on Agent Training and Support (CATS) has recommended that a standard evaluation form be used to assess the quality and utility of training offered to county Extension agents.  In response to these recommendations, a team of specialists was appointed to develop a common evaluation instrument suitable for this purpose.  The resulting evaluation form contains five questions about the content, seven about the quality of instruction, two about outcomes, and three questions that can be customized by the instructor.  Below are the instructions for using the form.

1. Access the form at  http://www.ca.uky.edu/agpsd/inseval.pdf

2. The evaluation form is presented as an Adobe Acrobat .pdf form.  The instructor should insert the name of the in-service session, the date, and name of the instructor.

3. The instructor may also customize questions 15-17 to ask specific questions about their session.

4.  Print the completed form and make enough copies so that each person attending the session can have one to complete.  It may be beneficial to print the copies directly to a laser printer.  This will help prevent misalignment of the response fields and reduce scanner error.  If the form is duplicated on a copier, please make sure that the original is correctly aligned on the stage and the margins on the copies are identical to the original.  Additional questions may be placed on the back, but do not staple additional pages to the form.  Stapled pages cannot be fed through the scanner.

5. At the conclusion of your training session, give each participant a copy of the evaluation form to complete.  (In the event that there are multiple instructors with significant teaching responsibilities, it is recommended that participants complete a separate form for each instructor.) They may use either a pencil or a pen to complete the form.  Tell them to darken the circle corresponding to their preferred response.  Encourage them not to fold the forms since they will be fed into a sheet-fed scanner.

6.  After the in-service, return the completed forms to Chuck Corby in the Program and Staff Development Office, Room 710 Garrigus Building, 0215.

7. The forms will be scanned on a Fujitsu sheet-fed scanner and analyzed using Remark Office OMR.  Standard descriptive statistics will be generated for all questions found on the front page.  The instructor will be responsible for analyzing any additional questions placed on the back of the form.

8. Original forms and data summaries will be returned to the instructor.  A summary for each session will also be sent to the appropriate Department Chair or Assistant Director.
