Instructions for Using Evaluation Tool

Links to evaluations for individual modules follow these instructions.

We have provided for your use a simple, one-page evaluation for each of the ten programs in the “UK Extension's Universe of Possibilities” series.

These follow-up instruments measure post-session attitude and behavior change resulting from an educational experience with one of the ten modules, typically in a facilitator-led, face-to-face, group-learning situation.
You may wish to ask those participating in your group to self-address envelopes during your learning session. Explain that you will send a very simple follow-up questionnaire to determine what they learned and how they have applied it.

After participants return their questionnaires, the percent of respondents who answer yes to Item 1 equals the percent who have made positive attitude changes. Drawing from a sample of their explanations under item 1, you will be able to give examples of the exact types of positive attitude changes that were made.

The percent of respondents who answer yes to Item 2 equals the percent who have made positive overt behavior changes. Drawing from a sample of their explanations under Item 2, you will be able to give examples of the exact types of positive changes in practice that were made.
You can easily calculate the percentage of respondents who gained the knowledge and skills referred to in the individual statements under Item 4.
